A Heartford Health nurse practitioner visits your loved one in their home for regular primary care, monitoring changes and helping address health concerns before they become bigger problems. On-site, after-hours support is also available. Should your loved one want to keep their primary care physician, Heartford Health will work alongside their Physician, with twice-monthly nurse practitioner visits providing added support and our team coordinating communication between providers.

Scheduling & Follow-Up

Your Heartford Health Resident Navigator helps schedule primary care and recommended follow-up appointments, taking care of the calls, coordination, and details that can condume your time.

Connected Care

Heartford Health helps coordinate information across their care team, including primary care providers, specialists, pharmacy, physical therapy, occupational therapy and others, helping ensure everyone stays informed and nothing falls through the cracks.
